Providence Health & Services RN Navigator - Oncology in Walla Walla, Washington

Description:

Providence is calling an RN Navigator Oncology (Breast Cancer, Dayshift, 1.0 FTE) to Providence St. Mary Medical Center in Walla Walla, Washington. In this position you will:

  • Maintains collaborative professional working relationship with clinicians and administrative staff necessary to develop and implement a successful cross-continuum care management.

  • Participates in system-wide care management and other program activities as requested.

  • Assists in the implementation performance improvement programs for the care management of cancer patients.

  • Promotes and provides patient, family, staff and community education through use of expert clinical knowledge base.

  • Facilitates and monitors patients’ progress; consults with primary physician, surgeons, radiologist, oncologists (or designee) and other members of the care delivery team as needed.

  • Completes a comprehensive assessment of patient on admission, refers patient to appropriate support services.

  • Coordinates with multi-disciplinary care team the development of the patient’s clinical care plan.

  • Assists in the development of patient education programs and tools.

  • Facilitates appointments for consults and support services within established service standards, including social workers, dietitians, physicians, etc.

  • Assists patients in understanding their diagnosis, treatment options, and resources available, including educating eligible patients about appropriate clinical research studies and technologies.

  • Partners with patients, families, the interdisciplinary team, and community resources to provide well-coordinated, timely, compassionate, exemplary, interdisciplinary care.

Qualifications:

Required qualifications for this position include:

  • Associate's Degree in Nursing

  • Current licensure in the State of Washington as a Registered Nurse

  • Oncology Nurse Certified, within 3 years of Hire

Preferred qualifications for this position include:

  • Bachelor's Degree in Nursing

  • Certified Nurse in disease specific process
