Providence is calling a RN Pediatric Admissions and Case Management (Full-time/Variable Shift) to Providence TrinityCare Kids Hospice in Torrance, CA.

We are seeking a Pediatric Admissions and Case Management RN who will perform patient evaluations and admissions including the interdisciplinary collaboration in initiating the Plan of Care for the patient/caregiver/family. The Coordinator of Program Development/ Community Liaison TKC collaborates with referral sources in the preparation and transition of patient/caregiver/family to hospice care services.

In this position you will have the following responsibilities:

  • Perform patient/family presentations, evaluations and assessments, including a history and physical and all relevant data to determine appropriateness for admission

  • Assist facility personnel in identifying patients eligibility for the hospice benefit

  • Use health assessment data to determine nursing diagnosi

  • Assist the patient/caregiver/family, the Interdisciplinary group (IDG) and the facility staff, if applicable, in establishing the level of hospice care provided

  • Collaborate with the patient/caregiver/family, facility staff, discharge planners, and physicians in meeting nursing and related needs

  • Take patient care orders and assure appropriate people are notified and follows up with referrals to confirm that all services and products have been arranged

  • Collaborate with team regarding admissions, discharge home planning, and follow-up. Complete patient admission report on team report line

  • Attend hospital rounds in key areas and intervenes with hospice patients in the Hospital Emergency Department, as appropriate

  • Attend department staff meetings

  • Identify business opportunities and works with the Manager of Clinical Operations and Director of Provider Relations to pursue potential partnerships or contracts

  • Maintain productivity standards as set by organization guidelines

  • Develop, coordinate and facilitate education for referral sources

  • Collaborate with Manager of Clinical Operations if staff needed for presentation and or admission evaluation

  • Provide information on TrinityCare Hospice services to physicians, HMOs, insurance companies, discharge planners and other referral sources

  • Develop and maintain ongoing relationships with all of the major tertiary medical centers serving children in TrinityKids Care service area

  • Interact with customers in a personalized and professional manner which includes active listening, open communication, and flexibility in responding to defined needs in a timely and accountable manner

  • Demonstrate respect, compassion, and tolerance when indicated; give information and problem solves with competency

  • Keep knowledge current and practices within the employee, administrative and patient care policies of TrinityCare Hospice

  • Identify leads and nurture new referral sources and potential customers

  • Identify contract and partnership opportunities and collaborates with the Manager of Clinical Operations and Director of Provider Relations to pursue prospects

  • In collaboration with Manager of Clinical Operations, develop new programs and services for TrinityKids Care

  • Documentation / Information Management:

  • Complete, and submit accurate and relevant clinical information regarding patient’s condition and care given electronically or written as appropriate to organization guidelines

  • Record pain/symptom management changes/outcomes as appropriate

  • Communicate with Clinical Supervisor to coordinate the plan of care

  • Document contacts and appointments in an efficient manner on a monthly calendar and weekly log

  • Work in collaboration with the family/caregiver and Hospice IDG members to meet the needs of the patient/family/caregiver

  • Submit documentation for the patient medical record within the timeframe set by the organization guidelines

  • Leadership / Supervision:

  • Upon request of supervisor, provides education to referral sources

  • Provide ongoing education on a formal and informal basis to all accounts and sources, to enhance understanding and acceptance of hospice practice and facilitate trust and referrals

  • Increase TrinityCare Hospice’s visibility in service area by participation in targeted professional and/or community groups, speakers’ bureau or health fairs

  • Generate referral leads and monitors results on an ongoing basis

  • Integrate and maintain quarterly pro-active meeting with priority accounts and documents a summary of discussions, action plan and timelines

  • Model effectiveness in customer relations and fosters staff responsibility for customer service

  • In collaboration with Manager of Clinical Operations, participates in the ongoing education of staff

  • Responsible to ensure managed care customer needs for status updates are completed timely

  • Participate in orientation, education, development of team members as directed by supervisor

  • Development:

  • Assume responsibility for personal growth and development

  • Participate in appropriate continuing education, in-services, and conferences as may be requested and/or required by Manager of Clinical Operations

  • Performance Improvement:

  • Actively participate in the Performance Improvement Program and participate in and adhere to the Providence/TrinityCare Hospice Corporate Integrity Plan

  • Assess, monitor and preserve customer satisfaction with each contact, consult with customers in challenging situations, and document and report identified opportunities for improvement and achieved solutions

  • Budgetary:

  • Follow Core Value for being financially prudent

  • Ensure that arrangements for medications, supplies, equipment and other necessary items and services authorized in the Plan of Care are available

  • Adhere to budgetary constraints for expenditures related to assigned responsibilities

Qualifications:

Required qualifications for this position include:

  • Graduate of a school of nursing

  • Current RN licensure in the State of California

  • Current CPR certification

  • California licensed driver with an automobile that is in good working order and insured in accordance with state and/or LCMHS/TrinityCare Hospice requirements

  • 1 year recent acute care experience in an institutional setting

  • Excellent observation, verbal and written communication, problem solving and basic math skills

  • Nursing skills per competency checklist

  • Visual acuity and hearing to perform required nursing skills

  • Demonstrates ability to communicate clearly and effectively

  • Proven ability to be flexible and responsive to customer needs

  • Demonstrates working knowledge of computer applications, i.e. Microsoft Office and e-mail

Preferred qualifications for this position include:

  • Pediatrice experience strongly prefrerred

  • Bilingual in Spanish strongly prefrerred

  • Nursing experience in theory and practice in hospice/palliative care or home health
