Providence is calling a Case Manager RN (Per-Diem/Variable Shift) to Providence Cedars-Sinai Tarzana Medical Center in Tarzana, CA.

We are seeking a Case Manager RN who will be responsible for ensuring timely and appropriate high quality, individualized patient care during hospitalization and identifying delays in the treatment or inappropriate resource utilization for neonates and their families.
In this position you will:
Assesses, diagnoses, plans, implements and evaluates patients’ care. Develops, updates and coordinates the patient’s plan of care to achieve patient goals and to optimize outcomes and transitions across the care continuum. Monitors, records and communicates patient condition as appropriate. Demonstrates competencies in populations served and performs essential competencies as required by specific unit of hire. Facilitates Care Coordination, Transition of Care and Utilization Review for all assigned patients.
Effectively advocates for ethical and holistic care by partnering in care planning to promote the autonomy, dignity, rights, values and beliefs of those we serve.
Minimum of an Associate's Degree in Nursing
All Registered Nurses (RN) without a minimum of a Bachelor’s degree in Nursing (BSN) must obtain a Bachelor’s degree in Nursing or higher within three (3) years of hire
An RN with twenty (20) years of RN experience will not be needed to obtain a BSN
Unencumbered RN license in state(s) of hire, and wherever care is delivered. If RN holds a multistate license through the Nurse Licensure Compact (NLC), the RN must adhere to the requirements of licensure in the new state(s) of hire.
Minimum of one (1) year of experience within the past three years in an acute hospital setting as a Registered Nurse
Minimum of one (1) year experience as a Discharge Planner or Case Manager, successful completion of a minimum of 24 CEUs in Care Management within the past 12 months, OR successful completion of the Transitions in Practice (TIP) program for Care Managers.
Current LA City Fire Card within 30 days of hire.
Bachelor's Degree in Nursing.
National Certification, CCM or ACM, preferred.
Minimum of three years experience as an RN in the past five years
