Providence Health & Services RN - Palliative Care - Peds/Perinatal in Spokane, Washington
Providence is calling a Registered Nurse (RN) - Care Transition Coordinator (Palliative Care, Day Shift, 0.8 FTE (hours weekdays 8 am- 5 pm-days can vary) to Providence Medical Group in Spokane, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Be responsible for performing individual patient/family assessments which will include assessing the patient/family's complex medical , emotional, social, spiritual and financial needs.
Work with the Palliative Care Team to create a comprehensive care plan based on patient/family goals.
Be responsible for assessing and facilitating appropriate treatment per orders for patients who are at the end of life on "comfort care".
Regularly communicate with Social Services to help facilitate use of both Community and Providence based resources.
Facilitate communication between the patient, referring physician, palliative care physicians and both inpatient/outpatient support services providers.
Provide ongoing support to families engages in the bereavement process by making referrals or offering follow up as appropriate.
Required qualifications for this position include:
Graduation from an accredited school of nursing
Currently licensed as a Registered Nurse (RN) in the State of Washington
Current Basic Life Saving (BLS) certification required within 60 days of hire
Preferred qualifications for this position include:
Bachelor of Science in Nursing (BSN) degree
1 years hospital/acute care experience
1 years experience with pediatrics or perinatal population
Experience in providing patient and staff education
