RN Care Navigator - (Oncology and Hematology, Providence Medical Group) $7,000 Sign-on Bonus

$7,000 sign-on bonus for eligible external hires that meet required qualifications and conditions for payment.

The Oncology Nurse Care Navigator will work collaboratively with the healthcare team to assess, plan, facilitate, track, and monitor patients from all cancer sites. Works closely with the patient and family; makes appropriate referrals to meet the patient's individual, cultural and spiritual needs, and monitors and evaluates services provided. Works as a liaison to and in collaboration with patients, physicians, disease management liaisons, and other healthcare providers. Responsible for facilitating all activities and services across the continuum of care for oncology patients. Responsible for coordinating and liaising to design and align resources to support program initiatives, program development, development and maintenance of relationships with internal and external referring physicians, and community resources. Performs all duties in a manner which promotes team concept and reflects the mission and values of Providence Health & Services.

Qualifications:

Required qualifications:

  • Bachelor's Degree in Nursing

  • Current Washington State RN License

  • Oncology Certified Nurse (OCN) certification required within 3 years of hire or transfer into the position

  • 5 years of RN experience
