In this position you will:

  • • Manages patient records, appropriately and accurately transcribes information to the interdisciplinary Plan of Care. • Initiates the patient chart. • Maintains patient charts, reports stat orders, and keeps patient records current. • Files paper reports (i.e., lab, x-ray, EKG, other facility records, procedure room records, misc.) and other appropriate material in patients’ chart. • Requests prior medical records upon arrival of patient. • Ensures prior medical records are available in a timely manner.• Contributes to smooth functioning and well-equipped unit. Standards: • Orders supplies for the unit and keeps station stocked and organized. • Demonstrates fiscal responsibility in ordering and maintaining supplies and forms. • Communicates with care delivery team any delays that may disrupt services. • Assists with notifications, paperwork, and procedures for admissions, transfers, appointments, transportation, discharges and deaths. • Transcribes physician orders for new admits into the electronic medical record as well as assists with transcribing daily physician order changes/additions. • Assists with completion of Discharge Instructions in a timely manner in preparation for patient discharges. • Maintains clean and organized care team station.• Responsible for accurate, courteous, and prompt unit communication. Standards: • Answers telephone promptly. • Communicates phone messages / call courteously, accurately, and promptly. • Assists in answering nurse call system courteously, accurately and promptly. • Demonstrates willingness to seek appropriate resources for inquires. • Communicates in a calm, professional and discreet manner. • Assists in excellent communication with physicians, providers, patients & caregivers. • Greets, escorts, provides orientation to room and services.• Contributes to and promotes flexibility and professionalism within the unit. Standards: • Demonstrates ability to work with constant interruptions. • Plans, prioritizes and organizes work efficiently. • Willingly floats to other units and/or other shifts when the need arises, upon request. • Maintains composure and demonstrates professionalism. • Performs duties of a CNA if urgent need occurs. • Identifies and reports issues and concerns and provides recommendations for resolution.• Manages and maintains files and records. Standards: • Provides patient and/or family with information about the department environment. • Portrays calm, reassuring manner when assisting patients and families.• Performs other related duties as assigned. Standards: • Accepts assignments willingly. • Prioritizes workload to ensure timely completion of assignment. • Ask appropriate clarifying questions relative to the scope of assignment.

Required qualifications for this position include:

  • Required upon hire: Washington Nursing Assistant Certification License

  • Required upon hire: National Provider BLS - American Heart Association

  • Required 1 year Recent experience as a Health Unit Coordinator or experience in a similar position.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life.
