Providence Health & Services RN - Care Manager in Portland, Oregon
Providence is calling a Registered Nurse - Care Manager (0.2 FTE, Days) to Providence St. Vincent in Portland, OR.
The schedule for this RN Care Manager is every other Saturday and Sunday working 8am - 4:30pm.
In this position you will:
Be responsible for facilitating the patient's progress through the hospitalization from pre-admission through discharge and transition to the next care setting.
Work with patients/families, physicians, nurses, social workers and other health team members to enable each patient to progress towards care goals and be prepared for timely discharge or transfer.
Apply clinical expertise and knowledge of utilization management principles to care facilitation and discharge planning.
Fully partners with and promotes focused programs such as Utilization Management and Clinical Documentation Specialists.
Required qualification for this position include:
Bachelor's degree in nursing
3 years acute care hospital or community based experience
Strong clinical Background with recent relevant experience
Current Oregon RN license.
Preferred qualification for this position include:
Master's degree in nursing
2 years acute care hospital supervisory experience
Utilization review or discharge planning experience
Certification in Critical Case Manager(CCM), Accredited Case Manager(ACM), American Board of Quality Assurance and Utilization Review Physicians (ABQAURP)
