Providence is calling a Charge Nurse (RN) for the Center for Medically Fragile Children (0.9 FTE, Nights) at the Providence Child Center in Portland, OR
We are seeking a Charge Nurse (RN) that will provide unit/department leadership and supervises clinical staff. Provides and supervises professional, independent evidence-based nursing care for Child Center residents using the nursing process, consistent with an Oregon RN license. Under the Oregon Nurse Practice Act, the professional registered nurse is legally accountable for 'diagnosing and treating human responses to actual or potential health problems'. The Providence staff RN applies knowledge drawn from broad, in-depth education in the social and physical sciences in assessing, planning, ordering, giving, delegating, teaching and supervising nursing care that promotes patients' optimal health and independence. All Providence professional nursing practice requires a broad range of general nursing knowledge and competency. In addition, development and validation of knowledge and skills essential the assigned department, patient population and/or practice area are essential.
This RN participates in and leads clinical teams and may supervise or delegate to licensed practical nurses (LPNs), certified nursing assistants (CNAs), certified medication aides (CMAs), and other health care workers.
In this position you will have the following responsibilities:
DOMAIN: Diagnostic and Monitoring Function:
Use independent, critical judgment in all aspects of resident care delivery
Assure nursing assessments are completed in a timely manner, including admission, initial shift and ongoing assessments
Manage clinical information by:
Reviewing clinical findings [data]
Interpreting and transforming those data to information
Applying professional knowledge
Derive appropriate nursing diagnoses though critical information synthesis
Create and implements a nursing plan of care for patients, based on assessment synthesis, nursing diagnosis and the patient's unique needs
Provide leadership in the interdisciplinary patient care team in integrating the nursing plan of care into the patient's interdisciplinary plan of care
Coordinate, assign and supervise appropriate activities involved in the nursing plan of care to CNA's and other nursing care team members
Document all assessments, plans and interventions in the patient's clinical record
Respect and support the patient's Sacred Space through presence and creating a healing environment
Provide safe passage by ensuring safe care processes and healing environments throughout the stay
Provide comfort measures through selection and implementation of evidence-based strategies for pain and symptom management
Provide consistent emotional and informational support to residents and families
Tailor caring processes to the individuality and uniqueness of each resident and family
DOMAIN: Teaching/Coaching Function:
Assess the resident and family's capability and readiness to learn
Provide resident and family education that is appropriate to the health care setting and that facilitates recovery, health maintenance, health promotion or a peaceful, comfortable death
Teach, mentor and precept other nurses and interdisciplinary team members
DOMAIN: Administering and Monitoring Therapeutic Interventions and Regimens:
Determine most appropriate, evidence-based nursing interventions for the specific resident.
Implement evidence-based interventions according to the Nursing Plan of Care.
Protect vulnerable residents from complications by evaluating risks, monitoring for early signs of complications, and individualizing the Nursing Plan of Care based on the resident's vulnerability
Administer medications accurately and safely, and monitors and documents adverse effects, reactions, therapeutic responses, toxicity and incompatibilities
Document interventions and resident responses in the medical record
Evaluate effectiveness of the Nursing Plan of Care and alters as indicated
DOMAIN: Monitoring and Ensuring the Quality of Health Care Practices:
Consistently evaluate resident care interventions, effectiveness and efficiency of the resident's plan of care, and clinical and personal outcomes of the hospitalization
Constantly focus on improving nursing care quality by readjusting care strategies and priorities reflecting critical evaluation of patient care and outcomes
Advocate for resident rights and responsibilities
Follow evidence-based standards and practices and maximize safe resident care
Know about, complies, and translates to others internal and external regulatory resident care requirements
Participate in quality improvement and nursing research activities at the unit/facility level
Honor resident and family values even when they differ from the nurse's personal values
DOMAIN: Organizational and Professional Development:
Communicate in a positive and constructive manner
Work collaboratively with others, maintaining communication and behaviors that facilitate teamwork
Demonstrate flexibility and creativity in response to unexpected changes in workload and/or patient status within a shift
Reassign resources as a response if needed
Participate actively as an interdisciplinary team member, team leader and the unit leader
Incorporate conflict management skills in resolving issues within the clinical team
Coordinate, order, and meet multiple patients' needs by realigning priorities as needed
Recognize personal learning needs and sets goals for further clinical and professional development for self and others
Required qualifications for this position include:
Current, unencumbered Oregon RN license
Current Basic Life Support certification (BLS) for Healthcare Providers
National Certification from ANA -OR- Professional Organization in relevant field of clinical expertise strongly recommended
One (1) year of nursing with experience in care of medically fragile children
Preferred qualifications for this position include:
Bachelor's Degree in Nursing
Pediatric Advanced Life Support (PALS) certification
Charge Nurse experience
