Providence RN Care Coordinator, Structural Heart Program - IHI in Missoula, Montana


Providence is calling a Registered Nurse (RN) Care Coordinator, Structural Heart Program (Surgery and Recovery Services Cardiac, Days, 0.8 FTE) to Providence Medical Group Motana in Missoula, Montana. In this position you will:

  • Assess barriers to care and stressors.

  • Assess the ongoing care needs of patients through interaction with patients, families and members of the health care team. Coordinates and plans for care consulting interdisciplinary team members as needed.

  • Consult regarding cases that are presenting problems and/or experiencing significant deviations from the plan of care, conferring with the Medical Director/Department Director as needed.

  • Demonstrate appropriate application of age specific standards, policies and procedures and guidelines in assessing patients.

  • Document accurately, legibly and promptly patient assessment, care and response to treatment, education, changes in patient’s status, supply utilization and charges on approved forms, using approved abbreviations.

  • Exercise clinical competence in the hospital and continuum setting including, but not limited to assessment, comprehensive evaluation, interpretation of diagnostic data, coordination of patient care, and completion of plan of care.

  • Help patient navigate health care system.

  • Help patient set self-management goals. Focus on patient education and skill-building.

  • Help to implement care plan (may write care plans and treatment goals).

  • Keep patient actively informed of treatment, anticipated responses while ensuring all necessary informed consents are signed prior to providing non-emergency care.

  • Pre-planning of patient visits as part of care team (gather clinical data/focus on clinician reminders)Gathers patient experiences (medications, self monitoring, etc.) prior to provider visit.

  • Provide patient and family education on common topics including but not limited to wound care, dressing applications, compression therapy, diabetic foot care, venous disease, wound off loading and underlying disease processes.

  • Responsible for follow-up of referrals to specialists, other providers, and other agencies.

  • Responsible for patient reminders and follow-up of missed appointments/tests (outreach).

  • Responsible for utilization of patient registry.

  • Update plan of care to reflect changes in patient’s condition, response to therapies or education.

  • Utilize available resources effectively by appropriate consultation and collaboration to maximize effectiveness of care.

  • Work with other staff members gathering all necessary equipment and supplies to meet patient and physician needs.

  • Evaluate effectiveness of care by assisting with the development of PI indicators, collecting data, monitoring ongoing PI status, discussion of PI findings in unit meetings and makes recommendations for action plans.

  • Serve as a preceptor or educational resource for new employees and students.

  • Present in-services or other educational offerings as assigned.

  • Provide clear, concise pertinent patient reports/information.


Required qualifications for this position include:

  • Bachelor's Degree in Nursing

  • Current licensure in the State of Montana as a Registered Nurse.

  • Current Basic Life Support (BLS) card must be from the following authorized governing bodies: American Heart Association (AHA) - (BLS Provider), American Red Cross (ARC) - (BLS for Health Care Provider), or Military Training Network (MTN) – (Healthcare Provider) upon hire. Advanced Cardiac Life Support (ACLS) must be from AHA or MTN.

  • Cardiovascular Specialty Certification required.

  • Two (2) years cardiac clinical nursing.
