In this position you will have the following responsibilities:
The Wound Care Coordinator is accountable for direct and indirect quality of care for patients according to the individualized needs of the patient, following the nursing process and in accordance with the Oregon Nurse Practice Act. The coordinator oversees the coordination for outpatient wound care as well as serves as a resource for inpatient wound care and skin issues.
Managing a nursing practice that include patients requiring a broad spectrum of care by practicing good organizational and time management skills.
Recognizing and responding immediately and appropriately to potential emergency situations that arise.
Maintaining patient clinical records in accordance with regulatory standards.
Maintaining and enhancing the quality improvement of patient care. Maintaining a high level of creativity and leadership within a rapidly changing health care enviro
In this position you will:
o Utilizing the nursing process, assesses patient status, intervenes appropriately, and evaluates patient response.
o Establishes a formal written plan of care for referred patients, documents specific nursing orders, and develops goals specifying expected outcomes of intervention.
o Assesses health knowledge, learning abilities, and educational needs of the wound care patients and families and develops appropriate plans for teaching.
o Utilizes all available resources and consults as necessary with members of the health care team in planning and developing individualized teaching and discharge plans.
o Timely completion of all necessary documentation, including outcome of interventions (according to wound care charting standards), treatment protocol utilized, response to treatment - physical and psychological, status pre and post treatment, complications and actions taken.
o Functions as a knowledgeable resource person to the nursing staff in the areas of expertise, related to prevention and treatment of wounds of all types, and management of the incontinent patient.
o Assures that the specialty services and clinic operate within financial constraints and productivity standards.
o Coordinates scheduling of patient care in chronic, acute, assuring treatment protocols are implemented in a timely fashion, safely, and appropriately, according to indications/medical diagnosis.
o Works closely with Materials Management, the Wound Care SIG, and outside vendors as needed to coordinate availability of needed wound care products, and to improve quality of service to patients.
o Participates in product evaluation of new equipment and supplies related to areas of expertise.
o Assists with the development of nursing standards related to wound/ostomy care, including policies addressing clinic operation.
o Demonstrates advancement of professional nursing practice through both internal and external continuing education opportunities, membership in professional organizations, and participation on hospital committees.
Required qualifications this position include:
- Graduation from an accredited school of nursing.
Current registered nurse license in the State of Oregon
Certification as a Wound and Ostomy Certified Nurse or Certified Wound Specialist within one year of hire.
Valid CPR card.
- 3 years Nursing experience with advanced wound care experience preferred.
