We are seeking a RN Case Manager - Care Management who will be responsible for developing and managing patient care outcomes for an entire patient case load. The RN Case Manager may perform assessment, treatment or care for patients of all ages, including neonate, pediatric, adolescent, adult and geriatric, based on population focus. Duties also include complex discharge planning and utilization review.
The Nurse Case Manager must be competent to analyze patient data with an understanding of the cognitive, physical, emotional/psychosocial, spiritual, and chronological maturation process. The Nurse Case Manager is accountable for meeting clinical and financial outcomes for the patient within an appropriate length of stay, with the effective use of resources and within established quality standards. The Nurse Case Manager has the authority to direct care and collaborates with the patient/family, assigned nurse, and the health care providers to accomplish these outcomes.
In this position you will have the following responsibilities:
Assume 24 hour accountability for a patient caseload
Complete an expanded assessment of patient and family capabilities and needs as appropriate for the individual patient and patient population served
Use the nursing process to plan and evaluate nursing care in collaboration with the assigned nurse and charge nurse
Assess, identify and review critical elements of a patient’s hospitalization and significant other/family situation, to facilitate achievement of discharge outcomes, post hospitalization recovery, health maintenance and promotion
Call (and document) each patient following discharge to complete an assessment of the patient/family post discharge adjustment
Consult and collaborate with assigned nurse, and charge nurses across departments regarding coordination of services
Assess patient's progress through expected hospital course, mobilize resources, and intervene to assure that patient clinical and financial outcomes are achieved
Collaborate with the health care team to set clinical and financial standards for a selected patient population
Provide clinical expertise to influence the direction of program development
Participate, as requested by nurse manager/directors in interviewing, assessing and making recommendations regarding staff applicant hiring decisions
Contribute recommendations regarding staff performance to nurse manager/director (including commendation, promotion, or disciplinary action up to and including termination)
Participate in development of staff through mentoring relationship, informal teaching, and presentation of formal educational programs
Demonstrate leadership and participation in hospital/program/nursing committees, research, patient care conferences, or other professional activities
Act as a role model in the provision of nursing care, by collaborating with and supporting nursing staff
Plan and implement complex discharge planning needs
Perform all aspects of utilization review for their patient caseload
Required qualifications for this position includes:
Three (3) years of acute care hospital experience
Registered Nurse in the State of Oregon
Preferred qualifications for this position includes:
Bachelor's Degree in Nursing
One (1) year of home health, mental health, substance use, hospice, and/or utilization review
Progressive nursing leadership experience, including charge nurse role
