Providence Health & Services RN - Cardiac Rehab Readmissions Coordinator in Medford, Oregon
Providence is calling a Registered Nurse - Cardiac Rehab Coordinator to Providence Medford Medical Center in Medford, OR.
This position needs 5 years of cardiology/critical care
experience. The experience can be a mix of CCU, CVICU, ICU, ED, and Case
Management/Cardiology Office. I do want some ICU experience as assessment and
working off of protocols will be key requirements. The other ask is education
experience. They will be educating physicians, patients, staffs, and our
community. Presentations, community involvement, physician relationship
building, case management, and collaboration with C-Suite will be required.
On the COPD side, a working knowledge of the disease process
would be needed. We will train the right person on the COPD side. Very few RNs
are going to have detail pulmonary experience. That is more the range of
(R)RTs. The ICU and education requirements above will provide the necessary
base and Dr. Bradford should be able to help with this knowledge building.
In this position, you will:
Be in a multi-faceted position providing comprehensive services to patients throughout the continuum of care.
Be responsible for the seamless throughput of our heart failure, AMI, and pulmonary patients that are at high risk for readmissions and to optimize the care of our high-risk patients to best practice guidelines.
Collect, trend, and analyze aggregate outcomes data for the patient population to be used in identifying best practices and targeting performance improvement efforts is a key performance indicator for this role.
Have advanced knowledge and expertise to identify and implement improvement processes, and the ability to design, direct and implement teaching plans.
Require substantial clinical competency and specialized knowledge, judgment, and skill based upon the physical, developmental, psychological, environmental and spiritual principles.
Have self-direction, motivation, organization, and interpretation of information relating to policies, procedures, and programs.
Have internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers.
Have a goal to reduce readmissions through clinical, quality, and cost effective evidence based and best practice targeted interventions for an assigned caseload of at-risk readmission patients.
Required qualifications for this position include:
Bachelor’s degree in Nursing
Current encumbered Oregon State License as a Registered Nurse (RN)
Current American Heart Association Basic Life Support (AHA-BLS) certification
3-5 years of recent cardiovascular care experience or critical care experience
Preferred qualifications for this position include:
Master’s degree in Nursing
Experience in Case Management, or as a Clinical Nurse Specialist
