Providence Health & Services Palliative Care Social Worker - Palliative Care Team in Lacey, Washington
Providence Health and Services is calling for a Palliative Care Social Worker (MSW) to the Palliative Care team in Lacey, WA. This is a part time position (.8 / 32 hours) Day shift position.
The Palliative Care Social Worker is an integral member of the multidisciplinary Palliative Care Outpatient Team and will work with medically complex patients and their families to provide supportive whole person care by 1) collaborative development of plans of care that align with the patient's goals 2) advance care planning 3) psychosocial and emotional support for transitions, crisis, and quality of life issues 4) patient advocacy and 5) identification of and linking patients to resources. The Palliative Care Social Worker facilitates coordination and communication between the patient, members of the healthcare team, and family in the decision making process to minimize fragmentation of care throughout the patients's journey.

Familiar with a range of psychosocial interventions that can alleviate discomfort
Value the impact of ethnic, religious, and cultural differences
Understand the needs faced by members of special populations and their families (e.g. children; physically, developmentally, mentally, or emotionally disabled; institutionalized)
Demonstrate the ability to recognize signs of impending death and prepare family members
Able to support patients, families, and caregivers in anticipatory mourning, grief, and bereavement
Demonstrate empathy and sensitivity in responding to the pain, suffering, and distress of others
Adapt techniques to work effectively with individuals from different age groups, ethnicities, cultures, religions, socioeconomic and educational backgrounds, lifestyles, and differing states of mental health and disability
Demonstrate familiarity with bioethical considerations and legal issues such as the right to refuse treatment; proxy decision-making; withdrawal or withholding of treatment, including termination of ventilator support and withdrawal of fluids and nutrition
Required qualifications for this position include:
Masters Degree in Social Work
Three years (3 years) of social services experience with a health care related organization; experience should include a minimum of two years (2 years) in a multi-disciplinary health care setting
Licensed as an Associate Independent Clinical Social Worker (LICSWA)
Preferred qualifications for this position include:
Palliative Care Certification or eligible and working toward certification or willing to complete certification within 2 years of hire.

The multi-disciplinary Palliative Care outpatient team is made up of MD's, ARNPs, MA, MSW, RN Care Navigators, and a Pharmacist. The clinic is located within the Lacey Primary & Intermediate Care Specialty Clinic in Lacey, WA
