Providence Registered Nurse (RN) Palliative Care Services in Everett, Washington

Providence is calling a Registered Nurse (RN) Palliative Care Services (1.0 FTE, Variable) to work in Pmg Nw Wa Palliative Care at Providence Medical Group Northwest Washington in Everett, Washington.

The Registered Nurse – Palliative Care is responsible for facilitating palliative care interventions and end-of-life discussions and care as part of the inpatient Palliative Care team. S/he works under the supervision of the Palliative Care team providers. S/he will work independently and proficiently, provide direct clinical support, and education for Palliative Care Services in the Inpatient setting. Collaborates with Nursing, Spiritual Care, Case managers, and Inpatient and Outpatient care providers in the provision of quality Palliative Care. The Palliative Care Nurse facilitates palliative intervention/ end-of-life discussion and care, assists Providers of Palliative Care Services, and responds to referrals/consults in the Emergency Department (ED) and other locations. The Palliative Care Nurse identifies and triages patients that may benefit from palliative care. S/he educates/mentors nurses, clinicians, and non-clinicians in evidence-based palliative care. S/he is available to work collaboratively with Inpatient social workers in discharge planning, and works with the Palliative Care team to complete discharge planning for ED patients who are admitted following treatment and initial consult with the ED Palliative Care team.

In this position you will:

  • In collaboration with the Program Manager, sets goals and objectives for self and the Service. Implements action plans to meet defined goals:

  • Thorough knowledge of the principles and practices of nursing, palliative care and end-of-life care. Demonstrates technical expertise with skills and knowledge to utilize organizational design, health care law, and information technology.

  • Ability to understand, be receptive, and respond to change; alter process and procedures to optimize quality care for patients receiving palliative care and/or end-of-life and communicates this in action and word.

  • Demonstrates continuous active learning which contributes to personal and professional growth. Reflects on own performance and looks for ways to improve.

  • Functions as a specialist in Palliative Care:

  • Supports palliative care provider consultations for hospitalized patients needing acute palliative by obtaining patient history, relevant family background, legal surrogate information and clarification of patient’s/family’s understanding of illness and treatment options.

  • Facilitates or co-facilitates family meetings related to current and advance care planning.

  • Assesses, plans, implements and evaluates care of hospitalized patients who are on the Palliative Service.

  • Models strong communication Skills. Acts as a patient advocate:

  • Communicates with patient, family, agent, and interdisciplinary team members about pertinent clinical information, palliative care and end-of-life issues.

  • Identifies ethical/moral conflicts and refers to the Ethics Committee for assistance as needed.

  • Works in collaboration with others:

  • Collaborates with attending providers, hospital staff, consultants, and the interdisciplinary team regarding patient care treatment and services.

  • Participates in interdisciplinary meetings/rounds so that care involving management of complex end-stage diseases, pain/ symptom management, psychosocial, spiritual support, and caregiver support are provided according to established protocols.

  • Assists in development of policies, procedures and clinical guidelines.

  • Provides care appropriate to the patient specific population:

  • Assess data relative to age of patient, illness, readmissions.

  • Interpret data based on illness and age-specific parameters.

  • Plan, modify, and deliver care based on age of patient.

Qualifications:

Required qualifications for this position include:

  • Bachelor's Degree in Nursing

  • 2 years of clinical experience in a hospital acute care setting with a background in critical care, oncology, palliative care, or hospice nursing.

  • Actively work to complete Hospice or Palliative Care Nursing Certification within 2 years of hire.

  • Current Washington State Licensure as a Registered Nurse.

Preferred qualifications for this position include:

  • Master's Degree in Nursing, or similar field

  • Experience in Palliative care inpatient or outpatient

  • Experience in staff development, education, and precepting nurses and other clinicians

  • ELNEC Core Training and/or Hospice Education Network core curriculum training or equivalent educ/experience

  • American Heart Association Basic Life Support (AHA BLS).
