Providence is calling a Palliative Care RN Coordinator (Per diem/Day shift) to Providence TrinityCare Hospice in Torrance CA.
This position is primarily phone case management with the option to work from home.

We are seeking a Palliative Care RN Coordinator who will be responsible for coordinating the activities of all palliative care team members and organizing the weekly team conference. The RN Coordinator is the primary link to referring staff and physicians who are seeking the services of the palliative care team. The RN Coordinator initiates the response to a request for consultation, assesses the patient, and involves other team members as appropriate and coordinates care through community settings and hospice. In addition to the clinical duties, the RN Coordinator is responsible for educating staff about palliative care, maintaining data on all patients referred, and participating in quality improvement activities as it relates to palliative care.
In this position you will:
Role within the Interdisciplinary Palliative Care Inpatient Consult Team:
Coordinate the palliative care consultation team
At the request of physicians and only after specific physician orders for a palliative care consultation are written, initiate the consultation process. Identify and inform other team members (physician, social worker, and chaplain) that they are needed to perform the consultation
Keep patient data and statistics of palliative care inpatient consultations, community palliative care referrals and care, and hospice GIP as appropriate
Conduct a thorough nursing, functional, and psychosocial assessment on assigned patients; assess whether patient’s condition requires the immediate attention of the physician, coordinate family conferences, complete clinical documentation within organization standards
Collaborate with other members of the health team in providing continuity of care to meet the unique needs of the patient; provide patient information to the physician. Coordinate palliative services into community setting and through hospice care as appropriate
Participate in in-patient and community palliative care interdisciplinary team meetings, recommend course of treatment and monitor ongoing care
Provide liaison and information to physicians, discharge planning, and other departments regarding quality of care issues. Recommend appropriate action based on the patient’s condition
Assist family and patients regarding decisions including goals and values, withdrawal and withholding of interventions, non-beneficial interventions and resuscitation status
Educate the patient and family regarding diagnosis of medical condition and prognosis
Assist in the referral of patients to appropriate health facilities and community resources, directly participating in the community palliative care plan and/or hospice plan as appropriate
Provide education to nursing staff/nursing facility departments, and family members regarding the special needs of the patient and elderly population
Establish and maintain courteous cooperative relations with the public, patients and other personnel
Participate in the development and implementation of care maps, protocols, and education policies
Documentation / Information Management:
Demonstrate working knowledge of the Hospital Information System (EPIC and Suncoast Solutions)
Perform documentation in patient’s chart which is timely, accurate and concise per organization policies/procedures and regulations
Utilize computer based communications and data base system
Leadership / Supervision:
Act as lead resource person in palliative care to hospital staff, community medical professionals and families
Assist with orientation and preceptoring as assigned
Maintain complete confidentiality of all patient and hospital-related information. No breaches of confidentiality to patient, family, co workers of the public
Assume responsibility and accountability for own professional growth and practice
Participate and attend continuing education seminars. Inservices and meetings to meet RN requirements for maintaining current licensure
Attend departmental meetings as required
Participate in/conducts inservices as requested
Competency Charting/Performance Improvement:
Participate in CQI projects including correcting deficiencies identified as required
Participate in budget development and compliance
Current unencumbered California RN license
Eligibility for HPNA certification required
2 years RN experience in the acute care setting or transitional care setting
Basic computer skills
Bachelor's Degree in Nursing
