Providence Health & Services RN - Clinical Liaison in Ashland, Oregon


Providence is calling a Registered Nurse - Clinical Liaison (0.6 FTE, Days) to Providence Medford Medical Center in Medford , OR.

In this position, you will:

  • Represent Providence home health and hospice (Home Services - HS) at patient care facilities and be a major access point for all customers (to include, but not limited to: hospital discharge planners/case managers, potential patients and their family members, physicians, physician office staff, service inquiries, etc.)

  • Coordinate and arrange home care services between HS department and patient care facilities for home health and hospice associated with a clinical referral.

  • Possess strong communication and interpersonal skills.

  • Act as a clinical resource and educate hospital staff regarding the services offered by HS, and contribute to the patient care plan as it relates to the discharge plan.

  • Provide information to ensure a smooth transition for patients and their families following hospitalization.

  • Handle new referrals by coordinating prescribed services with the physician, payor, patient/caregiver, provider, and department to ensure that services are provided in a timely and coordinated fashion.


Required qualifications for this position include:

  • Current Oregon Registered Nurse (RN) license

  • Current Basic Life Support (BLS) Certification

  • Valid drivers license and driving record consistent with Safe Driving Policy 330 Incident Point Matrix

  • 1 year of full-time experience in home health or hospice

  • Experience in home care

  • Experience in positions that have required extensive customer service skills

  • Computer skills (basic keyboard, intranet)

Preferred qualifications for this position include :

  • Bachelor's of Science in Nursing (BSN)

  • 1 year of full-time experience in home infusion

  • 1 year of full-time experience in discharge planning in an inpatient setting
