Providence Nurse Navigator Heart & Vascular in Anchorage, Alaska
Providence is calling an Nurse Navigator Heart & Vascular (Cath Lab Day Shift, 1.0 FTE) to Providence Alaska Medical Center in Anchorage, Alaska. In this position you will:
Initiates a comprehensive assessment of patient/family needs. Collaborates with team members to ensure needs are met.
Collaborates with team members, the patient, the patient’s family and others as appropriate in the development of a biopsychosocial plan of care to meet the patient/family needs through the continuum of care including home health, rehab services, palliative care and other supportive care.
Provides feedback communication to attending physician/advanced practice nurse (as appropriate and with patient permission) on services provided through the navigation team.
Collaborates with appropriate team members to assure pre-certification approvals for diagnostics, procedures and treatment.
Promotes and role models collaborative practice and relationships with other health care professionals. Recognizes the role that the cultural diversity of patients and their families, employees, medical staff, volunteers and community members play in achieving productive and positive relationships.
Presents heart and vascular education information to community and professional colleagues through the Heart and Vascular Center and continuing education program.
Provides training, consultation, and assistance to facility personnel, as needed or requested.
Interacts, educates, and mentors volunteers and students as part of the navigation team.
Offers heart health, diagnosis and treatment education to community by phone, e-mail or on a one-to-one approach.
Legibly and electronically documents patient care activities, as appropriate, in an accurate, concise, and timely manner. Competent in the input and retrieval of information within the various forms of written and electronic documentation.
Collects, analyzes and presents data regarding monthly progress.
Consistently demonstrates and incorporates principles and policies of safety and infection control as defined by Hospital policy and procedure, National Patient Safety Goals and Emergency Response procedures. Complies with Hospital standards to assure continued compliance and regulatory requirements are met.
Required qualifications for this position include:
Associates Degree - Graduate of schoo lof nursing
State of Alaska Nursing License required
2 years Cardiac nursing experience
Excellent collaboration and team building skills
Articulate and responsive to colleagues
Effective conflict management skills
Demonstrated analytical skills
Demonstrated time management and priority setting skills
Demonstrates a high commitment to quality
Preferred qualifications for this position include:
Bachelors Degree - graduate of school of nursing
5 years Cardiac nursing experience
